Referral Program Rules

The rules of our referral program are easy to follow, making it simple to claim your rewards.

The referring individual must be a current patient who has had an appointment within the past year and has no outstanding balance.
Referrals must be new patients who are at least 18 years of age at the time of their first appointment.
Referrals must list your name as the referral source on their new patient paperwork.
Qualifying referrals must be made within the same year.
